Contact us for shipping costs and tax. Then, send the completed form with a Money Order or Cashier's Check to the address at the bottom of the form. (PayPal payments will be available soon.) You will also be asked to send a photograph which we will use to create the appliqué. If you send us an email with a photo, please be sure the image you send is LARGE ENOUGH. It should be approximately 8" by 8" and at least 200 dpi. In megabytes, it should be about 2-5 mb. A small jpg you pull from a website will not work. Please be sure to respect the copyrights of others. Your photo must be your own.
